Job Description for Registered Nurse Case Manager

• The registered nurse plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience.
• Provides direct care and case management for a team of Agency clients.
• Demonstrates proficient skills using OASIS to admit, transfer, re-certify, and discharge home health patient/hospice patients regarding physician orders.
• Coordinate and supervise LPN’s and HHAs in the delivery of patient care.
• Maintain compliance with agency policy and procedures.
• Follow Medicare and Medicaid regulations.

Position Qualifications

• Graduate of an accredited school of nursing.
• Can be LPN to RN bridge. Current licensure in state and CPR certification. Online CPR certification not accepted.
• Associate’s or bachelor’s degrees, with one (1) year of hospice/home health care experience preferred.
• Excellent observation, verbal and written. communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist and excellent organizational and time management skills.
• Licensed driver with automobile that is in good working order and insured in accordance with the organizational requirements.
• Physical requirements, with or without reasonable accommodation: prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.  EEOC.
